A: The Healthcare Communities National Results Sharing Site at results.healthcarecommunities.org is the main location/method for organizations participating in 'HRSA sponsored Quality Improvement Programs' to upload (data and narrative) reports. Organizations currently login to this website separately with a unique organizational identifier. For FQHCs this login is related to the health center UDS number. The results.healthcarecommunities.org website is accessible from the www.healthcarecommunities.org website under the Office menu by selecting the Results Sharing menu item. Collaborative teams across the nation use this web-based reporting website to upload their monthly reports. The data is compiled and aggregated on a monthly basis to produce useful information on health trends and show the impact of Collaborative methods on chronic illnesses in patient populations. Health centers use this information for benchmarking and collaborative learning process.

Q: What are the requirements to gain access to this site?

A: Use of the Site is Restricted to Organizations participating in HRSA Sponsored Quality Improvement Programs and the Staff that support those programs. If you are a member or employee of a participating organization, it is likely you already have a login to this site. Improvement Organization (Health Center) level logins are supplied for group use, one per organization. If you are a FQHC, your login ID is related to your UDS number. If you have any doubt, submit a Request for support by choosing Contact Us from the Help menu above.

A: If you believe that you meet the criteria outlined in question 2 above, please submit a request for support by choosing Contact Us from the Help menu above. Please include as much information as possible about your organization and in what programs it participates. If you provide Support to HRSA sponsored Quality Improvement Programs, please provide those details as well. Please be patient as in many cases the site administrators will need to secure authorization to give you access, so there may be a delay. Confirming authorized access might take anywhere between one to two days.

A: The National Results Sharing Site is the national repository of Improvement Organization (health center) reported quantitative data and narrative senior leadership reports. Quantitative data reported by the improvement organizations is aggregated on a monthly basis and that aggregated data is made available in the form of easily understandable Graphs for each Quality Improvement Program. The aggregate information shared using these graphical methods facilitate benchmarking and peer to peer collaboration within the learning community.

Q: Can I share my logins with others?

A: Please keep in mind the following guidelines and restrictions regarding sharing logins to the NRSS:

• Improvement Organizations provided with a login may share their login within the team members belonging to the same organization only. The login information for one organization should not be shared with a different organization.
• Quality Improvement Program Support Staff are given individual logins to the site and should not share those logins with others.
• Logins discovered to be used by unauthorized users/organizations will be deactivated immediately.
